Connected devices track your vitals between visits. Our care team reviews the data continuously — and reaches out if something shifts, before it becomes a bigger problem.

We provide primary care, mental health support, and behavioral health services — all coordinated by the same team. If you're dealing with anxiety, low mood, or cognitive changes alongside a physical condition, we address all of it.

Your other doctors stay in the loop

If you have a specialist or a physician you've been seeing for years, we coordinate with them directly. Your health information is shared securely so no one is working in the dark.

Your family stays informed

With your permission, we keep your family members up to date on your health. They don't have to chase down information — it comes to them.

What to expect when Altos Care visits

When your provider visits in person

Your Altos Care provider comes to your room or a private space in your community at a scheduled time. They'll review how you've been feeling, check your vitals, go over your medications, and talk through any concerns you have. There's no rush. At the end, they'll update your care plan and share next steps with you and, if you'd like, your family.

When your visit is by Assisted Telehealth

One of our medical assistants comes to your community, introduces themselves, and sets everything up. They take your vitals and make sure you're comfortable before the visit begins. Then your provider joins on screen — and the assistant stays with you the whole time. It feels like a normal visit. Someone is always physically there with you.

If you have connected monitoring devices, your care team is watching your readings every day. If anything looks off, we reach out before your next scheduled visit. You don't have to wait and wonder — we'll come to you.

Living in a senior community doesn't make feelings of loneliness, anxiety, or low mood any less real. For many residents, these experiences are closely connected to physical health — and addressing them matters just as much as managing a chronic condition.

Altos Care includes mental health and psychiatric services as part of our care model. Our providers evaluate and treat depression, anxiety, and behavioral changes — and coordinate with your primary care provider so nothing falls through the cracks. We also offer structured movement and activity programming designed for seniors, which has been shown to improve both mood and physical function.

These services are available in person or via Assisted Telehealth — and are covered under Medicare and Medi-Cal for eligible residents.

If you've ever ended a call with a parent or grandparent wondering whether they're really okay — you're not alone. That uncertainty is one of the hardest parts of having a loved one in a senior community.

Altos Care gives you a way to stay connected to what's actually happening with your loved one's health. With their permission, we share updates, flag changes, and make ourselves available to answer your questions. You don't have to be a healthcare expert to understand what we tell you — we speak plainly.

And when something changes — a shift in vitals, a new symptom, a medication adjustment — we reach out to you before you have to ask.

Questions we hear a lot

Not necessarily. If you have a doctor you've been with for years and want to keep that relationship, we can work alongside them — providing monitoring, mental health support, and care coordination without replacing your primary care. Many residents do choose to make Altos Care their primary care provider over time, but it's entirely your decision.
Our primary care, chronic care management, and remote monitoring services are covered under Medicare and Medi-Cal for eligible residents. Mental health and psychiatric services are also covered for most eligible residents. There is no cost to your facility, and we handle all enrollment and billing directly. Contact us and we'll help you understand your specific coverage.
That's exactly why we send our own medical assistant to every telehealth visit. You don't need to know how to use any device or app — our MA sets everything up and is right there with you throughout. All you have to do is show up.
With your permission, we share updates with designated family members and keep them informed when something changes. Your family doesn't need to chase down information — we proactively communicate. You control who receives updates and how much detail they see.
If you're being monitored through connected devices, our care team is watching your readings daily and will reach out if something looks off. If you have an urgent concern, you can contact us directly and we'll determine the appropriate next step — which may be an Assisted Telehealth visit, an in-person visit, or guidance to seek emergency care if needed.
Yes. Family members and legal representatives can contact us to ask about care for a resident. We'll explain what's available, check eligibility, and coordinate with the resident and their facility to get started. The resident's consent is always required before we begin care.
