How Assisted Telehealth works at your facility

Every Altos Care visit — primary care or psychiatric — follows the same model. We bring the care to the resident. Your team does nothing.

Our MA arrives

An Altos Care medical assistant comes to your facility at the scheduled time. They check in, introduce themselves to the resident, and set up the telehealth equipment in the resident's room or a private space.

Vitals are taken

The MA takes all necessary vitals — blood pressure, pulse oximetry, weight, and any other readings relevant to the visit. Data is recorded directly into the care record in real time.

The provider joins

The Altos Care provider connects remotely and conducts a complete clinical visit. The MA stays with the resident throughout — assisting with communication, answering questions, and documenting next steps. Your nursing staff is not involved at any point.

We currently serve facilities across the Bay Area in California. Our programs are designed to scale — whether your community has 20 residents or 200, the model works the same way.

Questions facility partners ask

Nothing. Our programs are covered under Medicare and Medi-Cal for eligible residents. The facility does not pay for our services, does not purchase any equipment, and does not incur any administrative costs. We handle enrollment, billing, and coordination entirely on our end.
Very little. We ask for access to the facility and an introduction to your DON or designated point of contact for clinical coordination. We do not pull nurses or CNAs into visit setup, vitals collection, or documentation. That's what our medical assistants are for.
We work alongside existing physicians when that's the right arrangement. In those cases, we provide monitoring, care coordination, and behavioral health services while sharing data with the resident's PCP. Some residents choose to transition their primary care to Altos Care over time — but we never require it.
We have our own clinical documentation platform and share data securely with outside systems. We are not dependent on your facility's EHR and do not require you to adopt new software. We adapt to your environment.
After a partner meeting and agreement, onboarding typically takes two to four weeks. We handle credentialing, enrollment setup, and facility orientation. Your team's involvement during onboarding is minimal.
Our remote monitoring flags changes in real time. When something shifts, our clinical team responds — by Assisted Telehealth or an in-person visit — to intervene before a condition escalates. If hospitalization is unavoidable, we coordinate the transition and follow up on discharge to reduce the risk of readmission.
