Altos Care partners with skilled nursing facilities and assisted living communities across the Bay Area — bringing primary care, psychiatric services, and remote monitoring directly to your residents, using our own clinical team.
Most telehealth and care programs require your CNAs or nurses to set up equipment, coordinate visits, and manage follow-up. Altos Care works differently.
Every Altos Care visit — primary care or psychiatric — follows the same model. We bring the care to the resident. Your team does nothing.
An Altos Care medical assistant comes to your facility at the scheduled time. They check in, introduce themselves to the resident, and set up the telehealth equipment in the resident's room or a private space.
The MA takes all necessary vitals — blood pressure, pulse oximetry, weight, and any other readings relevant to the visit. Data is recorded directly into the care record in real time.
The Altos Care provider connects remotely and conducts a complete clinical visit. The MA stays with the resident throughout — assisting with communication, answering questions, and documenting next steps. Your nursing staff is not involved at any point.
We integrate with your facility from day one — no equipment purchases, no staff training, no administrative overhead on your end.
Board-certified physicians, NPs, PAs, and psychiatric providers. Credentialed, insured, and Bay Area-based.
On-site for every visit. They handle setup, vitals, and facilitation — so your CNAs and nurses stay focused on their work.
We supply and maintain all telehealth and remote monitoring devices. Nothing for your facility to purchase or manage.
We handle Medicare and Medi-Cal enrollment and billing directly. No administrative burden on your billing department.
We manage specialist referrals, follow-up, and communication with outside physicians. One point of contact for your clinical questions.
Secure sharing of clinical data with families, outside physicians, and your care team through our platform. No new software required on your end.
These are industry-wide findings. As we build outcomes data with our facility partners, we'll publish our own numbers here.

Facilities that partner with Altos Care see measurable improvements in resident health outcomes and satisfaction — and the operational benefits are equally real. We work with your team, not around them.
We currently serve facilities across the Bay Area in California. Our programs are designed to scale — whether your community has 20 residents or 200, the model works the same way.
