Altos Care was founded on a simple observation: seniors in skilled nursing and assisted living communities across the Bay Area deserve better access to consistent, high-quality care — and the technology and clinical expertise to make that possible now exists.
Seniors living in skilled nursing and assisted living communities often receive fragmented, reactive care. Their primary care physician may visit infrequently or not at all. Behavioral and psychiatric needs go unaddressed for months. A change in vitals that could have been caught on a Tuesday becomes an ER visit on a Friday night.
Altos Care was built to close that gap. We bring primary care, psychiatric and behavioral health services, and continuous remote monitoring directly to where seniors live — using a blend of in-person visits and our own model of Assisted Telehealth, where our medical assistants come on-site so residents and facility staff never have to manage the logistics themselves.
We serve the Bay Area because that's where we're from and where we know the facilities, the communities, and the gaps firsthand. And we built the clinical team we built because this population deserves specialists, not generalists filling a vacancy.
We go where residents are. Whether that's an in-person visit or Assisted Telehealth with our own MA on-site, no resident has to travel to get good care.
